The RISE Healthy Communities Summit (formerly The RISE Summit on SDoH) is the premier summit for leaders advancing whole-person, community-based care. The three-day event brings together 400+ cross-sector leaders from health plans, providers, public health agencies, community-based organizations, government, philanthropy, and social-impact organizations. Topics include funding diversification, operational partnerships, lived-experience integration, team resilience, and responsible AI/data use.
